  5. a statement by you that the above information is accurate and that you are the copyright owner or authorised to act on behalf of the copyright owner.

  1. the name and where relevant company name of the person making the request;
  2. where relevant the company details (including place of registration, company number and registered address);
  3. the name of the publication in which the HMV material will appear; and
  4. a description of the material the applicant wishes to use.
